San Joaquin Health Care 2004 JPA Handbook


Introduction

The San Joaquin Health Care Joint Powers Authority (SJJPA) is proud to present this Employee Benefit Handbook. This handbook outlines your benefit options for the plan year October 1, 2004 through September 30, 2005.

In 1980, the SJJPA was established by San Joaquin County School Districts to improve the quality of the benefits available to participating school district employees and to better manage the program’s cost. The SJJPA Board of Directors has consistently focused on maintaining a very comprehensive benefit package while ensuring the financial stability and integrity of the benefit programs offered. On October 1, 2003 the SJJPA partnered with another California Schools JPA and expanded our benefit Plan options and achieved a greater savings in premiums.

Participation in the SJJPA benefit programs provides each employee with the opportunity to select medical coverage under one of the two HMO plans or one of the numerous Blue Cross PPO Plans. Additionally, employees may select dental coverage under a PPO or DPO plan, and your district will offer one of the two vision carriers. Employees who choose a PPO plan have access to a 24-hour nurse advice line, a Wellness Program and Employee Assistance Program. Retirees are eligible to participate in the PPO and HMO medical plans. The benefits offered by the SJJPA and SISC III are among the most generous benefits available to Northern California school district employees.

Please read this handbook in its entirety. In an effort to continue to ensure the stability and continuity of this program, the SJJPA has made some changes in how to access your benefits, as well as some changes to benefit design.

Remember, in order to be covered, you must complete your enrollment forms and turn them in by the date specified by your school district’s personnel department. If you are an R.S.E.M.P. retiree, you must return your enrollment form to Legacy Enterprises.
